Terminal deoxynucleotidyl transferase (TdT) is a unique DNA polymerizing enzyme (~58 kDa) found in B- and T-cell lymphoblasts or prelymphocytes. It contributes to antigen receptor diversity by synthesizing N-regions at Ig heavy chain and T-cell receptor gene junctions.
TdT-positive cells are mainly observed in thymus and bone marrow, with variable expression depending on T-cell maturity. Occasional expression occurs in tonsils, lymph nodes, and extranodal lymphoid tissues.
Immunohistochemical detection of TdT is valuable for classifying malignant lymphomas and acute leukemias, particularly in identifying pre-B and pre-T acute lymphoblastic leukemia/lymphoblastic lymphoma (ALL/LBL).

Terminal Deoxynucleotidyl Transferase (TdT) is a nuclear DNA polymerase that catalyzes deoxynucleotide addition to the 3′ hydroxyl ends of oligo- or polydeoxynucleotides without a template.
TdT serves as a specific marker for diagnosing and classifying acute lymphoblastic lymphoma/leukemia and is particularly useful for distinguishing lymphoblastic lymphoma from Burkitt’s lymphoma.
