Target Information

The H antigen serves as a precursor to the ABO blood group antigens, present in individuals of all common blood types. The Bombay phenotype (hh) lacks the H antigen on red blood cells, resulting in the absence of both A and B antigens. Unlike the O group, individuals with the Bombay phenotype produce isoantibodies against antigen H as well as A and B antigens. Transfusion with O-type blood can lead to complement-mediated lysis due to anti-H antibody activity. Therefore, Bombay phenotype individuals can only receive blood from other hh donors but can donate as though they were type O.

Some individuals with blood group A1 may also produce anti-H antibodies due to complete conversion of H antigen to A1 antigen. The H antigen and its deficiency in the Bombay phenotype are controlled by the H locus on chromosome 19, which encodes a fucosyltransferase responsible for producing the H antigen on RBCs. The H antigen is a carbohydrate structure primarily linked to proteins, with a minor fraction attached to ceramide moieties.
