BBOX1 encodes gamma butyrobetaine hydroxylase, which catalyzes the formation of L-carnitine from gamma-butyrobetaine—the final step in the L-carnitine biosynthetic pathway. Carnitine is essential for transporting activated fatty acids across the mitochondrial membrane during mitochondrial beta-oxidation.
