Cytokeratin 18 (Keratin 18, KRT18, type I cytoskeletal 18)은 낮은 분자량의 산성 케라틴(type A) 계열에 속하며, Cytokeratin 8과 함께 존재합니다. 위장관, 호흡기, 비뇨생식기, 내분비 및 외분비 조직, 중피세포 등에서 발현됩니다. 주로 비편평상피(non-squamous epithelia)에 존재하며, 대부분의 선암(adenocarcinoma) 및 관암(ductal carcinoma)에서 양성으로 나타나지만, 편평세포암(squamous cell carcinoma)에서는 음성입니다. 간세포암(hepatocellular carcinoma)은 Cytokeratin 8과 18만을 발현합니다. Cytokeratin 18 유전자의 돌연변이는 특발성 간경변(cryptogenic cirrhosis)과 관련이 있습니다. 두 가지 전사 변이(transcript variant)가 동일한 단백질을 암호화합니다. Cytokeratin 아형 발현 패턴은 다양한 상피성 악성종양의 감별 진단에 활용되고 있으며, 면역조직화학, 세포병리학 및 유세포분석(flow cytometry)에서 유용한 도구로 사용됩니다.
