Cytokeratin 18 (Keratin 18, KRT18, type I cytoskeletal 18)는 저분자량 케라틴의 산성(type A) 서브패밀리에 속하며, Cytokeratin 8과 함께 존재합니다. 위장관, 호흡기, 비뇨생식기, 내분비 및 외분비 조직, 중피세포에서 양성 반응을 보입니다. 주로 비편평상피(non-squamous epithelia)에 존재하며, 대부분의 선암 및 관암종에서 발현되지만 편평세포암에서는 발현되지 않습니다. 간세포암은 Cytokeratin 8 및 18만을 발현합니다. Cytokeratin 18 유전자의 돌연변이는 원인불명 간경변(cryptogenic cirrhosis)과 연관되어 있습니다. 두 개의 전사 변형체가 동일한 단백질을 암호화합니다. Cytokeratin 아형 발현 패턴은 다양한 상피성 악성종의 감별 진단에 유용하며, 조직 절편 면역조직화학, 세포병리학, 유세포분석(flow cytometry) 등에서 활용됩니다.
