Target Information
Tramadol is a narcotic analgesic used for moderate to severe pain and may be habit-forming. Tramadol and its O-desmethyl metabolite (M1) are selective, weak OP3-receptor agonists.
Opiate receptors are coupled with G-proteins that regulate synaptic transmission by activating effector proteins such as adenylate cyclase and cAMP. Opioids decrease intracellular cAMP by inhibiting adenylate cyclase, reducing the release of nociceptive neurotransmitters (substance P, GABA, dopamine, acetylcholine, noradrenaline).
Analgesic properties are attributed to inhibition of norepinephrine and serotonin reuptake in the CNS. The (+) enantiomer shows higher affinity for OP3 receptor and enhances serotonin release, while the (-) enantiomer preferentially inhibits norepinephrine reuptake via α2-adrenergic receptor stimulation.
