Prostate-specific antigen (PSA)는 237개의 아미노산으로 구성된 단일 사슬 당단백질로, 약 8%의 탄수화물을 포함하는 세린 프로테아제입니다. 주로 전립선 상피세포에서 생성되며, 면역조직화학적으로 전립선 분비 세포의 특수한 층에서 발현됩니다. 전립선 선암의 약 99%에서 PSA가 검출되며, 악성도가 높을수록 염색 강도가 약해집니다. PSA는 전립선 및 인접 기관(직장, 방광)의 상피성 암을 구분하는 데 유용합니다. NKX3.1 및 Prostein과 함께 사용 시 민감성과 특이성이 향상됩니다.
Target Information
PSA는 전립선 상피에서 생성되는 키모트립신 유사 세린 프로테아제(kallikrein family)로, 정액 내에 풍부하게 존재합니다. PSA는 전립선암 환자의 혈청에서도 검출되며, 주로 α1-antichymotrypsin(ACT)과 결합된 형태로 존재합니다. 전립선암에서는 양성 전립선 비대증보다 ACT 결합형 PSA 비율이 높습니다. PSA는 원발성 및 전이성 전립선암의 선세포 기원을 확인하고, 비전립선성 암과의 감별에 사용됩니다.
